Hyperdiluted Radiesse
Dilution changes what Radiesse does. Undiluted, it lifts and contours. Mixed with saline or lidocaine, the gel becomes softer and spreads more evenly, the volumising effect fades and collagen stimulation in the skin becomes the point of the treatment. Published consensus describes two levels.
Diluted, 1:1
1.5 ml of Radiesse with 1.5 ml of diluent. Some volume plus dermal remodelling. Consensus uses it for laxity in the mid and lower face.
Hyperdiluted, 1:2 and above
1.5 ml of Radiesse with 3 ml or more of diluent. Minimal volume: the aim is firmness, elasticity and skin quality, typically on the neck, décolletage and body.
In 2024 Merz received EU approval for Radiesse to treat moderate and severe décolleté wrinkles using a 1:2 dilution with saline, based on a randomised, evaluator-blinded study in 117 patients. The US label added the same décolleté indication in 2026.

Preparation points from the consensus
- Mix through a Luer lock to Luer lock connector, with at least 20 passes between the syringes so the product is evenly dispersed.
- Inject straight away: diluted CaHA separates quickly.
- The panel recommends that the person injecting prepares the dilution themselves.
- Diluent: sterile 0.9% saline, lidocaine, or a mix of both. Radiesse (+) already contains lidocaine.

A lower-priced CaHA: Urevs 4D Fill
Urevs 4D Fill is a Korean calcium hydroxylapatite filler, the same material class as Radiesse, at a lower trade price: £87.99 for a box of 2 x 0.8 ml (1.6 ml), which is £54.99 per ml against £79.20 per ml for Radiesse (£118.80 for 1.5 ml). That is about 31% less per ml, and unlike Radiesse it qualifies for volume discounts of up to 10%. It is a different product from a different manufacturer, not a version of Radiesse, so follow its own instructions for use.
